At Townhouse we strive to make the admissions process as smooth as possible. Placing a loved one in a skilled nursing facility can be stressful and troublesome. Our goal is to guide you along the way. We urge families to come for a tour to obtain a better feel of our facility. Once the decision has been made to admit a patient to our facility, they will be assigned to a social worker who will ease the initial transition into the facility and will continue to be there to literally and figuratively hold their hand throughout their stay at Townhouse. Your social worker will also facilitate the admission process:
We will initiate and maintain constant communication with the social workers and case managers at the hospital, clarifying the patient’s needs and working together to transition your loved one to the competent care of Townhouse.
After our nursing department evaluates each patients specific needs, we will create an individualized plan of care to make the most of their time here.
Upon completion of the Nursing assessment, rehab patients will be screened by representatives of the Rehabilitation Department. This usually occurs within 24 hours of admission. The focus of the initial evaluation is to establish their current functional ability, prior level of independence and personal goals. From the moment you enter our facility for short term rehabilitation, our primary focus is to work towards your safe discharge home. The amount of therapy provided is determined on an individual basis.
